Supported Living Program Coordinator

Salary: $60,000–$68,000/Year
Schedule: Monday–Friday, 12 p.m.–8 p.m. Rotating on-call schedule
About the Role: The SLP Coordinator is responsible for providing leadership and overseeing multiple Supported Living Program (SLP) sites to ensure consistent, high-quality service delivery. This role includes ensuring appropriate staff coverage, maintaining timely and accurate program and attendance documentation, and implementing quality assurance measures across all locations. The SLP Coordinator will mentor new Direct Support Professionals and lead the implementation of person-centered practices in alignment with DHS guidelines, the HCBS Settings Rule, and CQL (Council on Quality and Leadership) standards. This position reports to the Supported Living Program Division Coordinator.
Responsibilities:
- Supervise and support Direct Support Professionals; provide caseload and program coverage as needed,
- Train staff, ensure full compliance with regulatory standards and training requirements,
- Lead/participate in trainings, committees, and task forces,
- Manage workload and submit weekly, monthly, and quarterly paperwork on time,
- Maintain accurate billing records, incident reports, and financial documentation (SNAP, petty cash, trust funds),
- Ensure adherence to all policies including scheduling, medication administration, staff payroll, and staffing plans,
- Provide on-call support on a rotating basis, and weekday consultation as needed,
- Support QA efforts and ensure completion of Plans of Correction and incident follow-ups,
- Collaborate with HR on staff recruitment, retention, onboarding, and compliance with labor policies,
- Coordinate with agency departments to ensure participant health, safety, community engagement, and promote human rights and dignity of risk,
- Participate in quality enhancement planning and goal execution as required,
- Ensure proper vehicle use and maintenance in collaboration with Transportation Manager,
- Support community integration, including Search for Adventure, special recreation, and Special Olympics,
- Maintain program environments and ensure site compliance with agency and state standards,
- Support participant well-being through medical care coordination, personal care assistance, and activity planning,
- Provide supportive counseling and crisis intervention as needed,
- Collect documentation of participant progress and daily records,
- Maintain agency and state standards on cleanliness, safety, and staff performance,
- Perform other duties as assigned.
Minimum Requirements:
- 3+ years of DSP experience,
- Maintain DSP certification,
- Strong communication and writing skills; ability to interact with nonverbal communicators,
- Knowledge of DHS, HCBS, and CQL standards,
- Valid driver’s license, personal vehicle, and insurance,
- Ability to pass literacy/medication administration test and meet Pace certification,
- Proficient in technology and documentation systems,
- Ability to lift up to 44 lbs. and perform basic physical tasks.
