Salary: $69,000-$73,000/Year
Schedule: Weekdays, 8:30a.m.–4:30p.m. with flexibility required based on program and participant needs. This position participates in on-call coverage and must respond to emergencies as required

About the Role: The Community Living Qualified Intellectual Disabilities Professional (QIDP) is responsible for providing leadership and overseeing multiple sites to ensure consistent, high-quality service delivery. This role leads the implementation of person-centered practices in alignment with DHS guidelines, the HCBS Settings Rule, and CQL (Council on Quality and Leadership) standards. This position has responsibility for timely and accurate completion of all service planning documentation, ensuring all medical needs are met for people on their caseload, and ensuring high-quality-standards for service delivery.

This position reports to the Division Manager of Community Living.

Job Responsibilities: 
  • Lead the creation and regular updates of Implementation Strategies (IS) in collaboration with the team, which includes the participant, family or guardian. direct support professionals (DSPs), and other relevant professionals.
  • Ensure that ISs reflect the participant’s preferences, goals, outcomes and needs, and that they comply with DHS, HCBS Settings Rule, and Council on Quality and Leadership (CQL) standards.
  • Supervise the implementation of ISs, ensuring that services are delivered as planned and that staff are properly trained.
  • Conduct regular reviews of program data and documentation to assess progress and make necessary adjustments to services.
  • Monitor and ensure the health, safety, and well-being of participants served, addressing any issues promptly.
  • Provide guidance and mentorship to Direct Support Professionals (DSPs), ensuring they understand and effectively implement the ISs.
  • Maintain regular communication with families and guardians to keep them informed about the participant’s progress and any changes to services.
  • Act as an advocate for the participant, ensuring that their rights and preferences are respected.
  • Ensure that all documentation related to services, including ISs, Risk Assessments, progress notes and incident reports are completed accurately and in a timely manner.
  • Stay informed about and comply with all relevant regulations and standards, including those set by DHS, HCBS Settings Rule and CQL.
  • Serve as a support in times of crisis, providing in-person support and intervention as requested.
  • Identify and address any barriers to service delivery, working collaboratively with the team to find solutions.
  • Lead, present, and participate in trainings, committees and task forces.
  • Provide on-call support on a rotating basis on weekends and weekly for emergencies.
  • Support community integration, including Search for Adventure, special recreation and Special Olympics.
  • Maintain program environments and ensure site compliance with agency and state standards.
  • Provide supportive counseling and crisis intervention as needed.
  • Perform other duties as assigned.
Minimum Requirements: 
  • Bachelor’s degree in human services
  • 1+ year of experience as a QIDP in Illinois,
  • Ensure CEs are completed annually to maintain QIDP certification,
  • Strong communication and writing skills; ability to interact with nonverbal communicators.
  • Familiarity with applicable regulations and standards, including DHS regulations, HCBS Settings Rule and CQL standards.
  • Valid driver’s license, personal vehicle and insurance.
  • Ability to pass literacy/medication administration test and meet Pace certification.
  • Proficient in technology and documentation systems
  • Ability to lift up to 44 lbs. and perform basic physical tasks.
